This episode is very loosely based off of chapters 54 and 55 of the manga.
The original backstory was that the Yggdrasil system was finally online again after the Lord of Terror arc as well as the gate destruction mandala fiasco, and Urd and Skuld had to go back to Heaven. Urd had to go back for remedial goddess training to reattain her license (the suspension had a note in her file after the Lord of Terror arc) and Skuld had to go because she had to get an Earth training license (although this is already done in the anime).
I have to say, I didn't expect Urd to have a copy of the Kama Sutra.
Belldandy originally got sick because she used a high-level spell against the Ninja in chapter 53, and her body was having an adverse reaction. Keiichi wanted to give her medicine, but didn't want to give anything to Belldandy he wouldn't give to himself. So by gulping down many of Urd's potions, he became a shemale.
So, basically, a good amount of the first half of this episode is original material. The visitation from the Auto Club people is new as well.
Originally, after Belldandy got sick and Keiichi turned himself into a shemale, that's when Keiichi attempted to call the Goddess Helpline.
Holy Bell appearing is actually a reference from the movie.
Originally, Urd gave instructions to Keiichi to create an antidote until she could get back. The Wind Crystals bit is new, as well as the Tokyo Tower bit.
Heck with it, almost this entire thing is original material. Originally, Keiichi had to give Belldandy the antidote mouth-to-mouth, and when Urd and Skuld got back Banpei had given Keiichi a "pervert" imprint on his face.
They kept in Urd's fascination with the size of Keiichi's breasts, although Urd's offer to make Skuld's bigger is new.
One interesting thing I caught in this episode was Skuld's mentioning that Urd would get a severe punishment for leaving her post. That could possibly be the license suspension that will lead up to the Lord of Terror arc.
